Utkal Hospital has marked another historic milestone in the field of advanced cardiac care by successfully performing a Transcatheter Mitral Valve Replacement (TMVR) — a first-of-its-kind achievement in the healthcare institution. This breakthrough further reinforces the hospital’s unwavering commitment to delivering world-class, high-quality healthcare to the people of the region.

The procedure was carried out on a 76-year-old male patient who had undergone mitral valve replacement surgery 12 years ago. He recently presented with severe mitral regurgitation (MR), a condition where the heart’s mitral valve does not close tightly, allowing blood to flow backward into the heart. Given his advanced age and lack of response to medical management, he was deemed unfit for another open-heart surgery.

In this challenging scenario, the heart team at Utkal Hospital, under the leadership of Senior Interventional Cardiologist Dr. Sushant Pradhan, decided to proceed with the TMVR procedure. The complex intervention was carried out successfully with the Cath Lab and CCU teams. Dr. Biranchi also played a vital role in ensuring the seamless execution of the procedure.

TMVR is a minimally invasive procedure that allows for the replacement of a damaged or diseased mitral valve without the need for open-heart surgery. It has emerged as a crucial option for elderly patients and those with high surgical risk due to its lower complication rate and faster recovery.

There are three main types of TMVR procedures — Valve-in-Valve (ViV), for previously implanted bioprosthetic valves that have deteriorated; Valve-in-Ring (ViR), for failed surgical mitral valve repairs; and Valve-in-Mitral Annular Calcification (ViMAC), for patients with significant calcification in the mitral annulus. In this case, the patient underwent a Valve-in-Valve TMVR due to degeneration of his previously placed valve.

Studies indicate that 20% to 35% of patients may require repeat surgery within 10 years of their initial mitral valve procedure. For elderly individuals, repeat open-heart surgery carries significant risks, with a mortality rate between 5% and 12%. TMVR offers a much safer and effective alternative in such situations.

Following the success of this pioneering intervention, Dr. Aditya Kumar Samal, Chairman of Utkal Hospital, along with CEO Mr. B.K.
